Hormone treatment, it has only highlighted the importance of maintaining a good level of fitness. Without the exercise I convinced that my mental attitude would have deteriorated too.

TALES FROM CHIAPAS Years spent as a member of Most Holy Trinity's Chiapas committee assisted in preparing me for my first visit to Chiapas in May 2003. I knew that our countless hours of fund raising and meetings were for a worthy cause and that I would be overwhelmed with the poverty and dire needs of the people. What I did not know, and quickly learned, was that their commitment to God and to each other has given them the perseverance and fortitude to fight for their human rights. I was overwhelmed by their fervent devotion, their humility, and their gratitude for all that the parishioners of Most Holy Trinity have done for them. I was even more overwhelmed by their eagerness to thank us by insisting that we join in their celebrations after shared Masses, weddings and First Holy Communions. We took piatas stuffed with candy to some of the villages to share in their celebrations. As the children participated in the fun, the wide-eyed expressions on their faces appeared to be symbols of hope: hope for a better future for them and for future generations. The piatas, meager soup bowls and tortillas shared with us, the music at times played on homemade instruments were also a reminder that fun and sharing are universal and a vital part of everyone's life. Participating in a hilly three-mile procession with the Chamulans was a humbling experience. As another Most Holy Trinity committee member and I slipped and slided in our Reeboks, we were passed by mothers nursing babies, toddlers and the elderly, all wearing shoes not fit to donate to St. Vincent de Paul. The procession culminated in a mass performed by Fr. Roberto and allowed time for me to thank God for my Reeboks! Drsmamine was much appreciated as we traveled over narrow rocky roads in ancient trucks and vans. These hilly roads are frequented on foot by the villagers, as they have no other means of transportation. During one meeting they humbly requested that we consider a financial loan to them. Forty-five families are attempting to farm, notably corn, on land in which approximately 00 in back taxes is owed. Without payment this land will be repossessed by the bank. The lives of forty-five families are dependent on this sparse land and yet they are unable to acquire about per family the approximate price of my Reeboks. Hopefully we will be able to finance this plea. Fr. Roberto, a giant of a saint, towers he is well over six feet ; over his vast congregation with an amazing love, strength and wit. He was our "tour guide" for much of the week and it was such a joy to see the happiness spread as he approached the villages. The children adored him and his playfulness. He is learning their very difficult Tzotzil language one more task amid multi-task days - and already celebrating Mass in this traditional language. The deacons are so very helpful, yet these Dominican missionaries are overwhelmed in providing spiritual, emotional and often financial guidance to their brethren. Seeing is believing. I have seen and I now support Most Holy Trinity's Chiapas project more than ever! Jeanne Olson June 4, 2003.

Relapse refers to the circumstance in which a patient becomes and remains culture-negative while receiving antituberculosis drugs but, at some point after completion of therapy, either becomes culture-positive again or experiences clinical or radiographic deterioration consistent with active tuberculosis. In such patients vigorous efforts should be made to establish a diagnosis and to obtain microbiological confirmation of the relapse to enable testing for drug resistance. True relapses are due to failure of chemotherapy to sterilize the host tissues, thereby enabling endogenous recrudescence of the original infection. In some hyperendemic settings, however, exogenous reinfection with a new strain of M. tuberculosis may be responsible for the apparent relapse 1 ; . Patients who are most likely to have true relapses are those with extensive tuberculosis whose sputum cultures remain positive after 2 months of chemotherapy 24 ; . Most patients relapse within the first 612 months after completion of therapy. In nearly all patients with tuberculosis caused by drug-susceptible organisms who were treated with rifamycin-containing regimens using DOT, relapses occur with susceptible organisms 5, 6.

Mike had headed off for a David Gerrold reading, but he didn't show up. Mike stayed in the room for the next reading, which was Hal Clement. I joined him for that. After that, I went to "Sixty Years of the N3F" featuring Catherine Mintz, Art Widner, Jacqueline Lichtenberg, and Helen E. Davis. This year is the sixtieth anniversary of the National Fantasy Fan Federation, which is especially useful to isolated fans and to all new fans. Mike went to "The Year in Physics and Astronomy" with Ctein, Jordin T. Kare, Dave Kratz, and Mark L. Olson. He reported that the room was packed. Many of the same topics were discussed as in the earlier science panel I had attended. We had dinner in the Marriott. There was a long wait to sit down and a long wait to order. I must have looked very displeased when we approached the cashier. The assistant manager asked if we were unhappy and offered us a voucher for a free breakfast the next day. By the time we got to the Hugo Awards, they were already starting. We were seated in the middle of the room, and I had to stand to take videos. Unfortunately, I ran out of tape towards the end. I went to a couple of parties afterward the ones for Charlotte and Boston. Then I went to the Fan Lounge, where I spent quite a bit of time talking to a fan from Detroit. After I got back to our room, I went back out to get some bottled water from a vending machine. However, the one on our floor was "Sold Out." I went on an elevator and punched the wrong button, but I got off at the first stop. It was the nineteenth floor. The vending machine there appeared to be working. I put in a dollar, which it accepted. Then it refused to take any more money, refused to give me any drinks, and refused to return my money. I went to the fourteenth floor, but the vending machine there was also "Sold Out." I decided to give up. It was after midnight, after all. I called Guest Services and was told I'd have to go down to the front desk for a refund. Then I ordered bottled water brought up. However, it had not materialized after half an hour and I decided to call it a night. I got up about 8h30 Monday morning with a bit of difficulty. However, we did want to make it down for the free breakfast. Since it was free, Mike ate. Before breakfast, I stopped at the front desk to get my refund from the vending machine. I also reported that I had never received the bottled water that I had ordered up to the room. The reply I received was a shrug, as though this was a routine occurrence. Since there was some time, we wandered around the exhibit hall looking at some photos of writers and other noted people. We also talked with Helen Davis for some time. Then I went to the "World Premiere Excerpts from Lois McMaster Bujold's Latest Unpublished Miles Book." Unfortunately, I had had to make a rest stop on the way. It took longer than I had expected, and there were no chairs left when I got to the programme room. I tried sitting on the floor, but I was so uncomfortable that I was unable to concentrate on the reading. It probably would have worked out better had I just stood leaning against a wall, but I that did not occur to me. I left. I ran into Nicki Lynch talking to a Philadelphia fan, and I ended up talking with them for about half an hour. Then I left for the next panel, which was "First Fandom Then and Now" with Forry Ackerman, Hal Clement, and Jay Kay Klein. We returned to the dealers room where Mike had earlier seen a skunk stuffy. We found the dealer again. She had several and some books about skunks. I made a large but fairly inexpensive purchase. We also had our final chat with Tom Kidd and his wife. Then we went back to the Marriott. After putting my purchases in our room, we went back downstairs for lunch. Service was very prompt. Many congoers had either left or were in the process of leaving. Then we went to the Closing Ceremonies. This was delayed a bit. The panel preceding it "The Field Since the End of World War II" with Greg Bear, Gardner Dozois, Esther Friesner, and George H. Schithers filled in the time until the tech crew was ready. The Closing Ceremonies were mostly pretty straightforward. Some reference was made to the Clarke-Kubrick 2001. We had dinner at the hotel. Between our main course and dessert, Jay Kay Klein joined us for some conversation. After refreshing myself back in the room, I checked out the dead dogs. Tuesday, 4 September I made a breakfast of soy nuts. I opted for sleeping in rather than ordering breakfast. We caught the 9h30 shuttle to the airport. Milt Stevens was with us. When we got to the airport, we found Bruce Pelz already waiting for the same flight. He had taken the earlier shuttle. While we were waiting, he managed to break his walking cane. He said he had another one at home and was not too perturbed. Our flight was smooth. We were well fed. Mike didn't eat anything except dessert, which was an ice cream sundae. I managed to catch about a half hour's worth of sleep. The downside of taking ginger instead of Dramamine is that ginger doesn't make me sleepy. I was pretty exhausted from the con and could have used more sleep. Instead, I read quite a bit. After we landed, I noticed that Bruce had hired a wheel chair to take him to the luggage retrieval area. The luggage was so slow in coming that an apology was delivered over the PA system.

Protect her airway. After awakening the patient, option include awake intubation, regional anaesthesia, and local infiltration anaesthesia. Continuation of general anaesthesia without significant foetal distress by face mask would be dangerous in obstetric patient as they are very much prone to aspirate gastric contents. The patient who cannot be intubated but who can be adequately ventilated by mask with foetal distress This is a situation where welfare of both the mother and child depend. Therefore, a delicate balance should be achieved to manage the situation. The initial step is to evaluate the adequacy of mask ventilation. Ventilation and oxygenation should be easy to maintain, rather than marginal, or tenuous upon which the following options depend. The first option, awake the patient.2, 21 This difficult decision would probably preserve the life of the mother, but it may result in the demise of the foetus. The second option is to continue anaesthesia via mask ventilation, while an assistant maintains cricoid pressure. When the lungs can be ventilated easily with a face mask after a failed intubation, it is better to use either a laryngeal mask airway LMA ; or an oesophageal tracheal combitube ETC ; in order to rescue airway.41, 42 However, the laryngeal mask airway LMA ; offers little advantage. The LMA may promote gastric regurgitation and may prevent escape of regurgitated stomach contents from the pharynx which "provokes" pulmonary aspiration. Application of cricoid pressure should be maintained unless it interferes with the maintenance of a clear airway. The proseal LMA PLMA ; may provide better protection against aspiration than LMA. Surgery can start when the patient is well anaesthetized using a volatile agent in 100% oxygen. Traditionally, halothane has been the agent of choice in this situation, however sevoflurane could be used due to its advantages over halothane. Sevoflurane is probably a more logical agent to use because of the ease of induction without breath holding and coughing, if available. It has similar properties of halothane in the spontaneously breathing patient, depth of anaesthesia is easier to control because of its low blood gas solubility and it is less cardio depressant. Fundal pressure and uterine exteriorization should be avoided to prevent regurgitation of stomach contents. Failed intubation drill Following induction of anaesthesia, if the anaesthesiologist fails to intubate the trachea, then a failed intubation drill should be initiated. There exist a multiplicity of failed intubation drills, some more complicated than others, but the essential requirement of all of them is to maintain oxygenation. If necessary, cricoid pressure. For example, on average 51 boys are born in every 100 births, so the odds of any randomly chosen delivery being that of a boy is: number of boys 51 number of girls 49, or about 04 equivalently we could have calculated the same answer as the ratio of the baby being a boy 51 ; and it not being a boy 49. Use of Pharmaceuticals in Combat Undoubtedly alcohol was the first drug to be utilized to enhance combat performance. When Holland became a major source of gin, the widespread use of this alcoholic beverage by soldiers led to the expression "Dutch courage" to express the desired effect. The ancient Assyrians, Egyptians, and Greeks reportedly utilized opiates before and during battles to sustain or enhance bravery and courage.53 Other drugs studied or used to enhance combat performance include ergot alkaloids, cannabis, amphetamine and other stimulants; Dramamine and other antihistamines; benzodiazepines; and L-tryptophan. It is the author's contention that the most extensive modern use of performanceenhancing drugs occurred among Soviet personnel during World War II shortly after amphetamine was synthesized. Amphetamine was useful not only to stave off fatigue and drowsiness but also to improve memory and concentration, particularly among Soviet pilots. During the Vietnam conflict, methylphenidate Ritalin ; and sometimes dextroamphetamine Dexedrine ; were standard issue drugs carried by long-range reconnaissance patrol LRRP ; soldiers. The LRRPs found the most efficacious use to be upon completion of a mission when fatigue had developed and rapid return to the base camp was desirable. Other than mild rebound depression and fatigue after the drug was discontinued, no adverse effects were reported. Other investigators54, 55 studying the drug abuse problem later in the Vietnam conflict reported problems with abuse of these stimulants. Although there was no documented abuse of the morphine Syrettes, commanders suggested such abuse might be occurring, 56 causing them to be withdrawn from the soldiers. Sedatives have also been studied as a method to improve performance in anxiety-producing situations such as paratroopers making low-altitude jumps or for reducing the emotional tension of young soldiers during the firing of guns.57 Reports of improved target accuracy through use of the adrenergic blocker, propranolol, and the anxiolytic, diazepam Valium ; , have resulted in a U.S. Army ban on use of these drugs by soldiers engaged in marksmanship competition because they would confer an unfair advantage. In the Vietnam conflict, neuroleptics antipsychotic or major tranquilizer drugs ; were widely utilized for psychotropic effects but benzodiazepines were also used. In the 1982 Falkland Islands.
