IVF Versus GIFT ZIFT TET If tubal disease is a factor then only IVF can be performed and GIFT, ZIFT, or TET are not an option. Because the fallopian tube is the natural site of fertilization and early embryo development, the tubal procedures are theoretically advantageous. However, many wellcontrolled studies have shown no advantage in using GIFT, ZIFT, or TET over IVF. The experience of many ART programs around the world now support the contention that IVF is the procedure of choice if the embryology laboratory is of the highest quality. Many programs have good GIFT, ZIFT, or TET success rates, but due to subtle laboratory factors, are less successful with IVF. Another major difference between GIFT and IVF, ZIFT, or TET is that with GIFT there is no documentation of fertilization. This procedure should therefore not be performed if a significant egg or sperm problem exists, or if there is any possibility that the fallopian tubes are not perfect. A major benefit of IVF to the patient is the avoidance of a laparoscopy and the general anesthesia required for GIFT, ZIFT, and TET. In September 2001 a "Dear Healthcare Professional" letter was sent out indicating 21 cases of acute angle-closure glaucoma had been associated with the use of topiramate. The authors of this report reviewed 115 new cases of spontaneous ocular side effects associated with topiramate therapy. They identified 83 cases of bilateral and 3 cases of unilateral acute, secondary angle-closure glaucoma. Ages of the patients varied from 3 years to 70 years of age. Eighty-five percent of the cases occurred in the first 2 weeks of treatment. Five cases were precipitated within hours when their dose was doubled. The most prevalent presenting symptom 38 patients ; was blurred vision. All the findings of acute glaucoma such as ocular pain, headache, nausea and vomiting, pupillary changes, hyperemia, and corneal edema were reported. Management of this condition requires stopping topiramate, instilling a topical cycloplegic and beta-blocker, and giving an oral intraocular pressure-lowing and detrol.

Neurostabilizers Neurostabilizers are an option for the continuous preventative treatment of migraines. As a drug class, neurostabilizers are 2.4 times more likely than placebo to decrease the frequency of migraines by 50% or more.41 Compared with placebo, they also reduce the number of attacks by 1.4 every 28 days.41 The exact mechanism by which neurostabilizers provide benefit in migraine prophylaxis is unknown. Anticonvulsants are typically dosed at much lower doses than when used for other conditions see Table 3 for more dosing information ; .22 Lower doses provide a better safety and tolerability profile. Variation exists among neurostabilizers. The number of studies examining these medications is small, and the number of patients in each study is also small. Tegretol activates certain enzymes in the liver. This causes Tegretol itself and many others medications to be metabolized faster. The serum Tegretol level may drop somewhat after the first month of treatment, requiring increased doses based on blood levels. The generic form of Tegretol, carbamazepine, appears to be less well formulated than the brand name drug. The tablets tend to fall apart and to be less well absorbed. In fact, it may reduce the intense food cravings that may occur with other commonly used psychiatric drugs such as Depakote, Risperdal, and Zyprexa. It can also be used as a sedating agent.
